This images comes from Google Street View which is where we saw this large blue plaque (the round one on the building, not the oblong one in front).  And that's the only place to see it, since when we visited in 2024 the building was almost entirely hidden behind construction wrappings, but we could see that the plaque had, wisely, been removed. Checking back with Google, the plaque appeared sometime in or before March 2017 (foliage totally obscures the view on dates before that) and was still there in July 2022.

The plaque is significantly larger than usual and we wondered if it might be the one for Notting Hill Carnival Pioneers, but comparing the two images one can see just from the layout that the text is different.

Quite possibly it's a purely functional board providing information to visitors to the Medical Centre. But we've marked the plaque as lost, hoping it will be re-erected once the building works are finished.  We will revisit.
